At a glance
Gynecomastia surgery removes the enlarged gland and fat that make the male chest look puffy or full, for a flatter, firmer contour. It is usually an outpatient procedure through small incisions hidden at the edge of the areola.
What it is
A procedure that removes firm breast gland and excess fat from the male chest, through a small incision at the lower edge of the areola, to flatten and firm the chest.
Where it is used
The chest: the firm gland directly behind the nipple and areola, and the surrounding fatty fullness, on one or both sides.
Anesthesia
General anesthesia, or local anesthesia with sedation for smaller cases, as an outpatient procedure. It usually takes one to two hours.
Downtime
Most men return to desk work within a few days, and to the gym by about four to six weeks, wearing a compression vest.
Results
A flatter, firmer, more masculine chest. The removed gland does not return, and the final contour shows as swelling resolves over the following weeks.
Who it suits
Men with persistent enlarged breast tissue that diet and the gym cannot shift, at a stable weight and in good health. A consultation confirms candidacy.
Why patients choose Tim Sayed MD
Good gynecomastia surgery is about a natural, masculine contour, not just removing tissue. The gland and fat must be removed evenly so the chest lies flat, the nipple sits naturally, and the scar stays hidden. Tim Sayed MD does exactly that.
Dr. Sayed is certified by the American Board of Plastic Surgery and the American Board of Surgery, with more than 20 years in practice.

He removes the gland and fat evenly and leaves a smooth layer under the skin, which avoids a dished or uneven chest.
He contours the whole chest so the result looks naturally masculine rather than over-flattened or dished behind the nipple.

He removes the firm gland by excision and the surrounding fat by liposuction, because treating only one leaves fullness behind.
The incision sits at the lower edge of the areola, where the scar is small and hard to see once healed.
If the fullness is mostly fat or weight-related, he will say so, and recommend the right approach rather than unnecessary excision.

Gland vs fat
True gynecomastia is firm glandular tissue behind the nipple; pseudogynecomastia is soft fat without excess gland. They can look similar but need different treatment: gland is removed by excision, fat by liposuction.
Both make the chest look full, but the cause differs. Firm tissue you can feel as a disc behind the nipple points to true gynecomastia; soft, even fullness that moves with weight points to fat. Most men have some of both.
Most men have a mix, and the plan treats both: excision for the gland, liposuction for the fat. The exam decides, and Dr. Sayed explains which is driving your fullness.

What it treats
Gynecomastia surgery removes the enlarged gland and excess fat that make the male chest look puffy or full, flattening and firming the chest and letting the nipple sit naturally.
It is not a weight-loss procedure and does not treat overall chest size from being overweight. Very loose skin after major weight loss may also need skin removal, which Dr. Sayed will discuss.
Matching the plan to the cause matters. Firm gland needs excision; fat needs liposuction; loose skin may need tightening. Dr. Sayed assesses all three and recommends what fits.

The procedure
Gynecomastia surgery is done under general anesthesia, or local with sedation for smaller cases, as an outpatient procedure, and typically takes one to two hours.
01
Dr. Sayed marks the chest with you standing, noting the gland and the fatty areas, then anesthesia is given for a comfortable, outpatient procedure.
02
A thin cannula removes the surrounding fat through a tiny incision, slimming the chest and blending the edges into the torso.
03
The firm gland behind the nipple is removed by excision through a small incision at the lower edge of the areola.
04
Dr. Sayed checks the contour and symmetry side to side, leaving a smooth, even layer so the chest looks flat and natural.
05
The incision is closed so the nipple sits naturally against the chest, and the fine scar hides at the areola edge.
06
A compression vest is placed to control swelling and support the chest, and you go home the same day with aftercare instructions.
Gynecomastia surgery is generally safe, but every procedure carries risk. Specific risks include bruising, swelling, numbness or changed nipple sensation, contour irregularities, asymmetry, fluid collection, and rarely a need for revision.
Dr. Sayed lowers risk with even removal, a smooth layer left under the skin, careful control of bleeding, a compression vest, and clear aftercare. He reviews your medications and health, since some causes of gynecomastia are treatable without surgery.
Surgery is deferred or declined when the fullness is from weight that is still coming off, when a medication or hormonal cause should be checked first, or in a teenager whose gynecomastia may still resolve on its own.
If losing weight first, stopping a contributing medication, or simply waiting is the better path, Dr. Sayed will say so.
Related and alternative options
Gynecomastia surgery treats the chest. Depending on your goals, it pairs with male liposuction of the flanks or abdomen, or body contouring, for a leaner torso in one recovery.
What fits best depends on whether the fullness is gland, fat, or both, and whether you want to address the flanks or belly at the same time. These are the related options.

Removes stubborn fat from the chest, flanks, and abdomen.
Often combined with gynecomastia surgery.
For fat without a firm gland.
Firms the torso and treats several areas together.
Includes skin tightening after weight loss.
A leaner, more defined torso.
Sharpens the jawline and firms the neck.
A masculine, natural facial result.
Also offered for men.
With male liposuction: the flanks and abdomen are slimmed in the same recovery as the chest.
With body contouring: a fuller plan firms the whole torso, including after weight loss.
Under-chin liposuction: sharpens the jawline, often with other facial work.
Skin tightening: when chest skin is loose after major weight loss, tightening is added.
The face: a male facelift or neck lift addresses the jawline when that is also a concern.
Before and after
Photographs show flatter, firmer, more masculine chests after gynecomastia surgery. Results vary from person to person and are shared with consent.
Photographs are examples of real outcomes and are not a guarantee. Individual results depend on anatomy, healing, and the specific plan.
Cost and financing
In San Diego, gynecomastia surgery commonly runs about $5,000 to $9,000, depending on whether one or both sides are treated and the anesthesia used. More involved cases cost more; the quote comes together after your consultation.
Gynecomastia pricing depends mainly on the extent of gland and fat, whether one or both sides are treated, and the anesthesia. The quote covers the surgeon fee, anesthesia, the facility, the compression vest, and follow-up.
Gland and fat: more tissue to remove, or both sides, takes more time.
Anesthesia: local with sedation usually costs less than general anesthesia.
One side or both: bilateral surgery treats twice the area.
Combined procedures: adding flank or abdominal liposuction is priced as a combination.
Surgeon experience: a double board-certified surgeon reflects training and a smooth, even result.

Recovery
Gynecomastia recovery is quicker than many men expect. Most return to desk work within a few days and to the gym by about four to six weeks, wearing a compression vest to control swelling.
Days 1 to 3
Soreness like a hard chest workout, bruising, and some swelling. Wear the vest, rest, and keep your arms gentle.
Days 4 to 7
Bruising begins to fade. Many men return to desk work toward the end of the first week, still wearing the vest.
Weeks 2 to 3
Swelling and firmness are normal and expected. Keep wearing the vest; the chest may feel numb or lumpy, which settles.
Weeks 4 to 6
Return to the gym as Dr. Sayed clears you, chest exercises last. The flatter contour starts to emerge as swelling drops.
Months 2 to 6
Swelling fully resolves and the final, flat contour shows. Numbness and firmness continue to improve and scars keep fading.
Wear the vest: it controls swelling and helps the chest settle flat; wear it as directed.
Walk early: gentle walking lowers clot risk and reduces swelling.
Ease back to the gym: chest and heavy lifting last, once cleared, to protect the result.
Be patient with swelling: the final flat shape takes a few months.
No smoking: smoking impairs healing; avoid it around surgery.
Stable weight: the removed gland does not return, and a steady weight keeps the chest flat.

FAQ
In San Diego, gynecomastia surgery commonly runs about $5,000 to $9,000, depending on whether one or both sides are treated and the anesthesia. The quote covers the surgeon, anesthesia, the facility, the vest, and follow-up; you get a firm number at your consultation.
Most men return to desk work within a few days and to the gym by about four to six weeks, chest exercises last. A compression vest is worn to control swelling.
Gynecomastia is enlarged male breast tissue, from glandular tissue, fat, or both, that makes the chest look puffy or full. Surgery removes the gland and fat for a flatter, firmer chest.
It depends on the cause. If the fullness is soft fat, liposuction alone may be enough; if there is a firm gland behind the nipple, that gland must be excised. Most men need both, which Dr. Sayed assesses at the exam.
The gland that is removed does not grow back. Significant weight gain, steroid use, or certain medications can cause new fullness, so a stable, healthy lifestyle keeps the result.
The main incision sits at the lower edge of the areola, where the fine scar is small and hard to see once healed. Liposuction adds only tiny, hidden incisions.
Cosmetic gynecomastia surgery is usually not covered. Some insurers cover it in specific medical cases; check with your plan. Dr. Sayed's team can provide documentation to submit.
The first few days feel like a hard chest workout, sore and bruised, controlled with medication. Most men are off strong pain medicine within a few days.
If a medication or a treatable hormonal cause is driving it, addressing that can help, and true gland rarely resolves on its own in adults. Soft, fat-only fullness can respond to weight loss. Dr. Sayed will tell you honestly whether surgery is needed.
Dr. Sayed reviews your history and medications and, when appropriate, may suggest checking for a hormonal or medication cause before surgery, especially in younger men or sudden cases.
Yes. It often pairs with liposuction of the flanks or abdomen, or wider body contouring, so the chest and torso are treated in one recovery.
Risks include bruising, swelling, numbness or changed nipple sensation, contour irregularities or a dished look, asymmetry, fluid collection, poor scarring, and rarely a need for revision. Even technique lowers these.
Adolescent gynecomastia often resolves on its own within a couple of years, so surgery is usually deferred unless it persists and causes significant distress. Each case is assessed individually with the family.
Good candidates are men with persistent enlarged chest tissue that the gym cannot shift, at a stable weight, in good health, and not smoking. A consultation confirms the cause and the plan.
