Tim Sayed MD is required by law to protect the privacy of your health information, to give you this notice of our duties and privacy practices, and to follow the terms of the notice in effect. We take this responsibility seriously.

How we may use and disclose your information

We use and share your health information to provide treatment, to obtain payment for services, and to run the practice. For example, we may share information with a surgical facility, a laboratory, or a financing partner involved in your care, and with our own staff who need it to serve you.

Disclosures that require your authorization

Most uses and disclosures not described in this notice require your written authorization, which you may revoke at any time. We will not use your information for marketing, or sell it, without your permission.

Your rights

You have the right to see and copy your record, to request corrections, to ask for a list of certain disclosures, to request confidential communication, to ask us to limit how we use or share information, and to receive a paper copy of this notice. To exercise a right, contact the practice.
