Breast Surgery
If heavy breasts are behind the ache in your back, neck, or shoulders, a reduction takes away the excess tissue, fat, and skin and lifts what remains into a smaller, better-proportioned shape. Tim Sayed MD, a double board-certified plastic surgeon, performs breast reductions for San Diego County patients at his La Jolla office near Westfield UTC, and when the operation is medically necessary, he gives you the paperwork to seek insurance reimbursement.

What it is
Surgery that takes away excess breast tissue, fat, and skin to relieve the strain of heavy breasts, then reshapes and lifts the breast and sets the nipple at a natural height.
Who it suits
Healthy women at a stable weight whose large, heavy breasts cause back, neck, or shoulder pain, grooves from bra straps, or rashes underneath.
La Jolla cost
Roughly $9,000 to $15,000 if you pay yourself, with the surgeon's fee, anesthesia, and accredited facility fees all included. Insurance may cover part of it when the reduction is medically necessary.
Procedure time
Roughly 2.5 to 3 hours under general anesthesia. It is an outpatient operation, so you recover at home that night.
Recovery
Back to desk work after about 1 to 2 weeks and to full exercise at around 6 to 8 weeks; swelling fades and the final shape settles over several months.
Provider
Tim Sayed MD, MBA, FACS: a double board-certified plastic surgeon in practice for over 20 years.
Augmentation adds volume, and a lift raises the breast without changing its size. A reduction works the other way: it takes out volume and weight to relieve symptoms, and lifts the breast in the same operation.
The procedure
The medical name is reduction mammaplasty. Dr. Sayed takes out the extra breast tissue, fat, and skin, reshapes the breast that is left so it sits higher and firmer, and moves the nipple and areola up to a natural position. The nipple stays connected to its own blood supply throughout, which helps protect sensation and, for many women, the ability to breastfeed. Expect about 2.5 to 3 hours under general anesthesia, and you go home the same day.
For most patients the goal is comfort, not looks. When breasts are too big for your frame, the weight drags on your neck, shoulders, and back, bra straps leave deep grooves, moisture gets trapped underneath and causes rashes, and workouts and getting dressed get harder. Taking that weight away usually relieves those problems, and because the breast is lifted and reshaped at the same time, it ends up higher and better proportioned.
Breast reduction is for women whose size is the problem. If you like your size but have lost lift after pregnancy or weight change, look at a breast lift; if you would rather add fullness, breast augmentation is the closer match.
Candidacy
Most good candidates have breasts heavy enough to cause real physical symptoms and want lasting relief, plus a smaller, lifted shape. Good general health and a steady weight matter too. When you come in to the office on Executive Drive, Dr. Sayed looks at your breast size, skin quality, and nipple position, records your symptoms, and tells you plainly whether a reduction suits your goals.
live with ongoing back, neck, or shoulder pain from the weight of your breasts
get grooves from bra straps, or rashes and irritation in the fold beneath the breast
are healthy overall and your weight has been steady
don't smoke, or can give up all nicotine for several weeks on either side of surgery
find your size gets in the way of exercise, work, or sleep, and have realistic expectations and time set aside to recover
plan to get pregnant later, since pregnancy can enlarge the breasts again and alter your result
are breastfeeding now or stopped within the last 6 months
are in the middle of a large weight loss and would do better to reach a stable weight first
are happy with your size and mostly bothered by sagging, in which case a breast lift fits better
would like more volume, not less, which is what breast augmentation is for
Because a reduction is partly medical and partly aesthetic, Dr. Sayed is direct with you at the consultation: how much tissue can safely come out, where the scars will sit, and what insurance is likely to do if your symptoms qualify.
Your options
Unlike most aesthetic surgery, breast reduction is often treated as medically necessary. If heavy breasts cause documented symptoms, some of the cost may be reimbursable, so it is worth knowing how coverage works before you come in.
Chronic pain, grooving, or rashes that conservative measures have not fixed can make a reduction medically necessary. A reduction done mainly for appearance is cosmetic and self-pay. Plenty of patients land somewhere in the middle, and the consultation sorts out which route applies to you.
Expect your carrier to want three things: documented symptoms, proof that conservative care such as physical therapy or supportive bras was tried for several months, and a minimum amount of tissue removed, set by your body surface area. Dr. Sayed takes those measurements and records them at your visit.
Insurance is not billed directly for breast reduction. Instead, Dr. Sayed's office gives you the itemized procedure and diagnosis codes and the operative documentation to submit to your carrier for out-of-network reimbursement. Medicare patients may need to sign a self-pay waiver.
Policies vary, so bring your insurance information to the consultation. Dr. Sayed will document your symptoms and walk you through both the medically necessary route and the self-pay route.
Relief matters as much as appearance here. If heavy breasts cause genuine symptoms, good documentation can support an insurance claim for part of the cost.
How it works
Your reduction takes place at an accredited San Diego surgery center under general anesthesia and usually lasts about 2.5 to 3 hours. Dr. Sayed marks your skin while you are standing, then removes the excess tissue, fat, and skin quadrant by quadrant. The nipple stays on a pedicle of tissue that carries its blood supply while he lifts it and the areola into place, reshapes the smaller breast, and closes the incisions in layers. Your result comes down to three choices: the incision pattern, how the nipple stays attached, and how much is taken from each side.
Liposuction only. Small incisions and liposuction reduce volume in breasts that are mostly fatty and barely sag.
Lollipop. For moderate reductions: one scar around the areola and one running straight down to the crease.
Anchor. The lollipop scars plus a third along the crease. It is the usual choice for large reductions and marked sagging.
Pedicle. The nipple is lifted while still connected to its blood supply and nerves, which helps keep sensation and often the ability to breastfeed.
Areola size. If the areola has stretched, it is usually made smaller to suit the new, higher breast.
Symmetry. Few women start with matching breasts, so each side gets its own measurements and plan.
Free nipple graft. In a rare, very large reduction the nipple may be removed and grafted back on; Dr. Sayed has needed to do this only a couple of times.
Amount removed. Enough tissue comes out to relieve your symptoms, but not so much that the blood supply or a natural shape is put at risk.
Combined procedures. When it is safe, other procedures can be done during the same operation.
Not sure reduction is the right operation? A breast lift treats sagging without excess size, breast augmentation adds fullness, and the full range of breast surgery at Tim Sayed MD covers everything else.
Cost
In the San Diego area, self-pay breast reduction generally runs about $9,000 to $15,000 all in, which covers the surgeon's fee, general anesthesia, and the accredited facility. If documented symptoms make the surgery medically necessary, insurance may reimburse part of it and bring your out-of-pocket cost down. Because the price depends on how involved your reduction is, the written quote you receive after your consultation is the only exact number.
Budget about $9,000 to $15,000 if you are paying yourself. Insurance may reimburse some of that when your symptoms meet your carrier's criteria; the written quote from your consultation gives the exact figure.
Complexity. A bigger reduction or more reshaping means more time in the operating room, and the fee reflects that.
Technique. An anchor reduction is a larger operation than liposuction alone, so it costs more.
Anesthesia and facility. Your quoted price already includes board-certified anesthesia care and the accredited surgical facility.
Surgeon experience. You are choosing a double board-certified plastic surgeon with over 20 years in practice, and that experience can help make results more predictable.
Medically necessary reductions come with the codes and documentation for an out-of-network claim, and monthly financing can spread out any self-pay balance. To get a feel for costs before you visit, see our plastic surgery pricing.
Recovery
Many women notice the weight is gone as soon as they wake up. Desk work usually resumes in about 1 to 2 weeks, and full exercise around 6 to 8 weeks. Swelling is heavy early on and eases over the next few weeks, while the final size and shape take several months to show. You leave with a written recovery plan, and Dr. Sayed sees you at each stage.
Day of surgery
Home the same day, wearing a supportive surgical bra, with medication for soreness and swelling.
First week
Take it easy and keep arm movement limited. Soreness, swelling, and bruising reach their peak, then start to fade.
Weeks 2 to 4
Back to desk work and light daily routines. Keep wearing the surgical bra Dr. Sayed recommends, and skip underwire.
Weeks 4 to 6
Add activity step by step as your follow-up visits allow. Full workouts and heavier lifting usually return around 6 to 8 weeks.
3 to 6 months
Most of the swelling is gone, scars keep fading, and the final smaller, lifted shape shows.
Sleeping. For the first few weeks, sleep on your back with your upper body slightly raised to protect the healing breasts.
Driving. Plan on about 10 days without driving; you can get back behind the wheel once you no longer need prescription pain medication and can move comfortably.
Exercise. Start walking early. Chest and upper-body training waits until you are cleared, around 6 to 8 weeks.
Scars. Incisions start out pink and firm, then soften and fade over 6 months to 2 years, with scar-care guidance from Dr. Sayed along the way.
Breastfeeding. Since the nipple stays attached to its tissue, many women can still breastfeed afterward, but it cannot be guaranteed.
The surgeon
A good reduction depends on judgment: knowing how much tissue to take while keeping a safe blood supply and a natural shape. Tim Sayed MD holds board certification from both the American Board of Plastic Surgery and the American Board of Surgery, trained at UCSF, Harvard, and Massachusetts General Hospital, and has more than 20 years in practice. He plans your surgery, performs it himself, and follows your recovery.
He is also known nationally for breast implant removal and breast implant illness care, work that depends on the same careful handling of tissue, blood supply, and incisions as a reduction. Look through his credentials and results before you decide.

Life in San Diego moves outdoors, from La Jolla Cove and Torrey Pines to year-round beach days, Pilates, and coastal hikes, and breast reduction is planned to make all of it more comfortable once you heal. Because the coastal sun can darken fresh scars, Dr. Sayed asks every patient to keep incisions covered and out of direct UV for the first several months; good sunscreen makes that easy.

Questions
In the San Diego area, a self-pay breast reduction typically comes to about $9,000 to $15,000, which includes the surgeon's fee, general anesthesia, and accredited facility fees. If documented symptoms make it medically necessary, insurance may pay back part of that. Dr. Sayed's office supplies the codes and documentation for an out-of-network claim, and you get an exact written quote after your consultation.
Frequently, if it is medically necessary. Carriers may pay part of the cost when heavy breasts cause documented pain, grooving, or rashes that conservative treatment has not resolved. A reduction for appearance alone is cosmetic and self-pay. The office does not bill insurers directly, but it gives you the procedure and diagnosis codes and operative notes to file for out-of-network reimbursement.
Most carriers look for documented symptoms (back, neck, or shoulder pain, grooving, or rashes), proof that conservative care such as physical therapy or supportive bras was tried for several months, and a minimum amount of tissue removed based on body surface area. Dr. Sayed measures and records each of these at your consultation so your claim is complete.
Many women feel lighter right away. You go home the same day with a written plan, avoid driving for about 10 days, return to desk work in roughly 1 to 2 weeks, and are usually cleared for full exercise around 6 to 8 weeks. The final shape appears over several months as swelling settles.
Often, yes, because the nipple stays connected to the tissue, ducts, and blood supply beneath it. No breast operation can promise breastfeeding, though, and sensation or milk supply may change. Let Dr. Sayed know if nursing in the future is important to you so he can plan around it.
Tissue that is removed does not grow back, so most patients keep their relief and smaller shape for many years as long as their weight stays stable. Pregnancy, significant weight gain, and ordinary aging and gravity can still change breast size over time.
Taking off the extra weight usually relieves chronic back, neck, and shoulder pain, the grooves from bra straps, and rashes or irritation under the breasts, and it makes exercise, posture, and finding clothes easier. Many patients feel the difference as soon as they wake up.
No fixed age applies. Teenagers with severe symptoms are sometimes candidates, although surgeons generally prefer to wait until the breasts have finished developing, and healthy adults of any age can be candidates. Dr. Sayed talks through timing and development at your consultation.
Every reduction leaves a scar around the areola. Depending on the technique, it may also run down to the crease and sometimes along it, while liposuction alone leaves only small marks. Scars start pink and raised and usually soften and fade over 6 months to 2 years. Dr. Sayed places them where bras and most swimwear cover them and guides your scar care, but they never vanish completely.
Most patients describe soreness and swelling for the first several days rather than sharp pain, and the medication Dr. Sayed prescribes usually keeps it manageable. You go home the same day in a supportive surgical bra, most people stop prescription pain medicine within a few days, and many say the relief from the weight outweighs the discomfort of healing.
Start with board certification from the American Board of Plastic Surgery, a surgeon who does reductions regularly, and before and after photos of their own patients. Ask who actually operates, who prepares your insurance documentation, and who sees you during recovery. Tim Sayed MD is double board-certified, has more than 20 years in practice, and sees every patient himself, from the consultation at the La Jolla office through healing.
